1. Scope of Services
Element 369 Diagnostics provides home sample collection and processing for pathology and health diagnostic tests across Mumbai locations, with operations based in Juhu, Mumbai.
- Lab Processing: All biological samples collected by our phlebotomists are processed through our accredited partner laboratories (NABL / CAP certified).
- Service Availability: Home collection slots are subject to phlebotomist availability, location coverage within Mumbai, and suitable weather or traffic conditions.
2. Booking, Scheduling & Cancellations
- Accuracy of Information: You agree to provide accurate personal details, including full name, age, address, contact number, and relevant medical history or doctor prescriptions required for the requested tests.
- Preparation Requirements: Patients are responsible for following specific pre-test guidelines (such as overnight fasting or medication restrictions) provided at the time of booking. Failure to adhere to guidelines may require test rescheduling.
- Cancellations & Rescheduling: You may cancel or reschedule your home collection appointment up to 2 hours prior to the scheduled time slot without penalty.
3. Pricing & Payments
- Transparent Pricing: All diagnostic package rates and collection charges displayed on our platform are final and inclusive of applicable taxes unless stated otherwise.
- Payment Terms: Payments can be made online through our secure payment gateway or via Cash/UPI upon home sample collection.
- Refunds: Refunds for cancelled appointments (where eligible) will be processed to the original payment method within 5–7 business days. No refunds will be issued once a sample has already been collected or processed by the lab.
4. Test Reports & Delivery
- Turnaround Time: Test report timelines (typically 12–24 hours depending on the specific test) are estimated. While we strive to deliver reports promptly via WhatsApp or email, occasional delays due to lab verification protocols or technical issues may occur.
- Medical Disclaimer: Diagnostic test reports delivered by Element 369 Diagnostics are intended for informational and diagnostic reference only. Reports do not constitute medical advice or a final prescription. Always consult a qualified medical doctor or physician for clinical diagnosis and treatment decisions.
5. Patient Responsibility & Sample Integrity
- Phlebotomists reserve the right to refuse sample collection if safe, hygienic working conditions are not provided, or if a patient exhibits aggressive behavior.
- You agree to inspect that sample vials are properly sealed and barcoded during your collection appointment.
6. Intellectual Property & Website Use
All content on this website—including logos, graphics, text, and layout—is the property of Element 369 Diagnostics and protected under Indian copyright and trademark laws. You may not copy, reproduce, or redistribute any material without prior written permission.
7. Governing Law & Dispute Resolution
These Terms of Service are governed by and construed in accordance with the laws of India. Any legal proceedings, disputes, or claims arising out of our services shall be subject to the exclusive jurisdiction of the courts located in Mumbai, Maharashtra.
